It was an honor and a pleasure for us to have had you as a guest at the two days of advanced training for the 5th Oralplant National Congress, an update and technical improvement meeting aimed at innovating scientific research in the implantology field.

This year's focus was on the evolution of implant techniques in complex bone regions and, in support of this theme, courses and conferences were dedicated with important personalities in the sector, who alternated in the interventions by exposing the innovative procedures and the effectiveness of the results obtained in the excellence of osseointegration and aesthetic prosthesis.

A global space in an open space which has attracted and intrigued the guests for its setting.

A multimedia exhibition and a display area with the latest news in implantology served to map out the guidelines for Oralplant methodology with specific and detailed treatment of the issues.

Chaired by Professor Pietro Oscar Carli and by Dr. Claudio Di Chiara, during this intense day anticipated by another one dedicated to intensive courses, were held the speeches of the experts with the aim of further exploring the conference's issues. Through the results demonstration and the analysis of clinical cases over time, has emerged and has been proven “on-site” the effectiveness and the innovation of Oralplant osteointegrated systems.

The subjects took turns, from the more strictly methodological to the technical ones related to materials, procedures, experiments and restorations, until the confirmation of the beneficial effects on bone regeneration even in cases of extreme rehabilitation. The day ended with the exposure of the developments and of the objectives Oralplant deemed to pursue in the near future.

The speakers have demonstrated the effectiveness of titanium membranes due to their properties and their simultaneous usage with that of implants; in fact, a study showed an excellent success in bone regeneration and an implant survival near 100%. If one of the disadvantages of resorbable membranes can be represented by the shift from the bone defect, it is shown that the titanium biocompatible membranes VBR maintaining their original shape perfectly close the defect always remaining in contact with the bone tissue and the results obtained had demonstrated the perfect autologous bone regeneration in 4 months.

He presented the results collected on a group of 13 Oralplant professionals who have used 68 Tuber-Plant S implants on molarsl. The case studies reported the 94.12% success (64 implants) and only 5.88% of failure (4 implants). Another randomized data collection, carried out in April 2016, demonstrated, however, in all the 487 implants inserted from 2010 by 126 Oralplant professionals, a success rate of 94.87% (462 implants), a failure rate of 2.46% (13 implants) and an unjustified failure rate of 2.67% (12 implants).

She dealt with the implant surface TPSS explaining how the bone regeneration is performed, unlike other implants available today on the market, from the surface to the bone as well as from the bone to the surface, significantly reducing the treatment times. Finally she examined the various types of action on surface, additive or subtractive, demonstrating the effectiveness of Oralplant particular technique which uses the subtraction by micro-tool to make the surface uniformly rough with an high degree of purity without deposit of foreign particles, called TPSS (Titanium Pull Spray Superficial).

One of the key to long-term success of implant treatments is the health of soft and hard tissues, though specifying that the attention to domestic and professional oral hygiene is not always sufficient to head off the risks of perimplantitis. There were presented the results obtained from comparative studies on genetic predisposition to perimplantitis; in particular, a screening through the innovative Array CGH technique (Comparative Genomic Hybridization) allows to immediately identify any chromosomal abnormalities and to obtain a definitive and accurate diagnosis.

He dealt with osteointegration stability (primary and secondary) over time in relation to the bone surface and implant shape, especially the conical one in post-extraction surgery, demonstrating that the stability obtained when positioning the implant is purely mechanical and is basically influenced by the bone characteristics, by the implant shape and the operator's skills; in order to restore an implant, the osteointegration is no more a prerequisite, but it is the primary stability.

He explained how the short implant has become an implant supporting the conventional ones, both in the upper zone and in the lower zone, demonstrating that positioning mistakes have a negative influence on the result and on implant's persistance. He showed that implants are stable and have no bone resorption for over 6 years; thanks to Short implants have been solved clinical cases that before were impossible to be realized with other implant types.

Ha analizzato come le connessioni coniche siano migliorative rispetto a quelle ad esagono esterno, sia dal punto di vista biologico che biomeccanico. Negli impianti con connessioni BCCO, si possono osservare presenze ossee tra gli impianti anche a distanze molto ravvicinate tra di loro, addirittura fino al moncone. Attraverso l’utilizzo di una mascherina chirurgica ed una fresa di diametro standardizzato, si può agire a livello crestale e mantenere le distanze corrette per posizionare l’impianto e garantire il successo della protesi e della salute dei denti circostanti.

He evaluated the benefits of immediate and delayed load demonstrating how the optimal solution is to act with an Oral-kone BCCO implant of high-biological productivity with Overdenture and explaining the success factors of the conical connection: no micro movement between implant and abutment, bacteria-proof connection between the implant and abutment, platform switching, subcrestal implant positioning, micro-roughness up at the implant/abutment interface.

She analyzed the microbiological, histological and mechanical aspects, with experimental demonstrations, of the various types of commercially connection (screwed, cemented, conical), demonstrating how the latest, to which belongs Oralplant BCCO system, are able to withstand in an optimal manner to bacterial leakage, because they don't generate microgap between implant and abutment. In the in vitro study of BCCO connection implants, it is rated as the tourque used for the implant/abutment closure, can influence the presence of bacteria. Optimal results are achieved with the use of a 40N torque.

She presented the results of a multicenter study carried out in 14 years, saying that thanks to the reduced peri-implant bone resorption and the very high survival rate is possible to use both the Tuber-Plant and Bio-Plant systems for implant rehabilitation, which has in fact reported a success of 97.53% (on 123 considered implants). Another study showed finally how Koni-Plant implants with polished collar and microtreated crestal collar have bringing the full success rate (on 83 recalled patients).

He has exposed the Oralplant Ti-CER and Ti-CER CE system of abutments, which allows the transfer of the location of the connection (hexagonal as regards the first one and conical for seconds) and the implant base during the detection of the optical impression. The same abutments can also be used as a basis for the cementing of personalized artifacts in zirconium oxide. The system is finally completed by the scanning body that is placed directly on the abutment and used for the monitoring with camera.

He has exhibited the new components CIR (Carico Immediato Ripetibile - Repeatable Immediate Load), consisting of a impression transfer Op-SFIL Lean (broken down into three pieces and reusable), an healing cap (which follows the implant trend), an occlusal stop limit (for each single type of screw), a pillar and an extractor for abutments (already treated and liftable by 10 mm in height). The Op-SFIL system is a well-known Oralplant patent already for many years.

He spoke specifically of the maintenance of dental instruments with regard to sterilization and disinfection stages, a very important process not only for the protection of the patient but also for the protection of the employees, assistants and dentists. He studied the regulations and procedures to maintain inside the clinic, to avoid the risk of contamination by infectious diseases, high enough risk in the clinic, as using sharping and stinging tools. For the insertion of implants are used instruments for implantology but not limited to, surgical instruments are also used to prepare the seat, cut the tissue and suture, with the aim of achieving a good functional aesthetic work, but always paying attention to risk of illness for themselves and, for DT. He has exhibited different content for business development in dentistry, affirming the importance of team management in various areas. He has addressed the topic "the power of the shares", ie the power to get involved and to influence or govern in some cases a result to ensure that the outcome is in charge and somehow brought home. He interacted with the group, answering their questions "how can I?", to have a different result, how can I do my job better, how can I get what I maybe already get today but more simply and with more serenity. Ask yourself a question, a very simple question that shifts the focus, how can I do to get this thing?

He has exhibited several tools for communication and management within the clinic. To relate in a different way, to communicate between people of the same Staff and improve communication with Patients, optimizing both the timing and results. It combined a practical part with a theoretical one by connecting the participants with each other, putting them to the test.

THE SCIENTIFIC RESEARCH CONFIRMS
THE HIGH QUALITY OF ORALPLANT

MCS Multicenter Clinical Study Oralplant

Analysis and research, thus confirming implant treatment, are the basis of the Oralplant MCS comparative studies; the comparative review of the results obtained is an important parameter for the scientific Implantologist.

To date, Oralplant's Multicenter Clinical Studies MCS-01/01 and MCS-01/10 have involved the participation of 30 professionals who, through the use of Bio-Plant, Tuber-Plant, Koni-Plant and Koni-Plant M implants, have been able to evaluate, with step by step clinical protocols, the survival rate of prosthetic implants. Periodic checks on duly informed and involved patients have allowed the drawing up of a final report on 14 years of follow-up through the MCS-01/01 research showing a success rate of 97.53% on 78 Bio-Plant and Tuber-Plant implants used, with an average bone resorption not exceeding 1.09 mm, for the Bio-Plant type and not more than 0.58 mm for the Tuber-Plant type (see publication on “Quintessenza Implantologia” 2012; 3:37-44). More positive results are also confirmed by the new Multicentre Clinical Study MCS-01/10 started in 2010, with Koni-Plant implants, with collars polished with OP-Lapping and Koni-Plant M implants with collars subject to Microtreatment with circular machining and “TPSS-Light”. This latest research is aimed at comparing the two types of collar and the rate of survival of the implants, that to date is 100% for the first 124 rooted implants.

MULTICENTER CLINICAL STUDY MCS-01/01 (on 14 years)

At 14 years’ distance from the functional insertion of the implants it was possible to recall 49 patients from the same study cohort for a total of 81 Bio-Plant and Tuber-Plant implants, in order to radiographically evaluate the peri-implant bone resorption and implant survival rate. A successful implant survival of 97.53% and an average bone loss of 0.86 mm.

MULTICENTER CLINICAL STUDY MCS-01/10 (on 4 and 5 years)

4 and 5 years after functional insertion of Koni Plant and Koni-Plant M systems it was possible to recall 83 patients from the same study cohort to evaluate the first results of success and survival rates. It was confirmed a successful implant survival rate of 100% and an average bone loss of 0.72 mm on Koni-Plant implants and of 1.08 mm on Koni-Plant M.

Tuber-Plant S and Tuber-Plant SC Implants in 5.0 – 5.7 – 7.0 mm diameters and in 2.5 – 3.5 – 4.5 – 5.0 – 6.0 mm lenghts

Were presented the results obtained from our data collection given by our implant specialists, who appreciated the solution with Short and Extra-Short implants, in 6 to 2.5 mm lenghts, even in extreme and particularly complex situations. During the event the results have been supported by posters dispayed in the conference area, which highlighted the innovative peculiarity of the solutions.
